Every service is designed to keep your loved one supported and your family informed.

Appointment Companion

A trained support person meets or accompanies your loved one at appointments, helps them stay organized, takes general notes with permission, and sends a clear summary to the authorized family contact.

Wellness Check-In

An in-person or telephone visit focused on general well-being, immediate practical concerns, safety observations, food access, and upcoming appointments. Not a medical assessment or emergency service.

Transportation Coordination

We help identify, arrange, or coordinate transportation for appointments, errands, and approved activities through vetted family, taxi, rideshare, or partner options.

Grocery & Essential Errands

Assistance with planning, ordering, pickup, delivery coordination, or accompaniment for groceries, pharmacy pickups, and essential errands. No medication handling.

Family Update Summary

With appropriate consent, the authorized family contact receives a written summary after each visit: date, observations, tasks completed, concerns, and follow-up items.

Post-Hospital Transition Support

Non-medical support after hospital or rehabilitation discharge — checking that follow-ups are scheduled, transportation is arranged, and the family understands the written plan.

Families living abroad often need someone locally to check in, accompany a relative to an appointment, help coordinate errands, or send a reliable update home. Jamaica services will begin in selected service areas and expand as vetted coordinators become available.

Service availability, travel fees, transportation costs, and response times depend on the client's parish and location.

Sharon P. Tulloch, MSHA, CPHQ is a healthcare professional, nurse, care manager, transitional care coordinator, and healthcare administrator in training. Her professional experience has shown her how easily older adults, individuals with chronic conditions, and their families can feel overwhelmed by appointments, transportation needs, follow-up responsibilities, and communication gaps.

Clarity Care Coordination is her response—a warm, organized, non-medical support service designed around the way real families live today: across cities, across borders, and across time zones.
