Appointment Companion
A trained support person meets or accompanies your loved one at appointments, helps them stay organized, takes general notes with permission, and sends a clear summary to the authorized family contact.
Clarity Care Coordination provides trusted, non-medical support, appointment companionship, wellness check-ins, and family updates for loved ones who may need an extra layer of assistance.
Serving families locally and across borders, with Jamaica and Central Florida service options.

Many families live far from aging parents, relatives, or loved ones who need practical support. You may worry about missed appointments, transportation, food, home safety, loneliness, or whether important information is being communicated correctly.
Clarity Care Coordination helps bridge that gap through trusted, organized, non-medical support and clear family communication—so the person you love is looked after, and you're kept in the loop.
Every service is designed to keep your loved one supported and your family informed.
A trained support person meets or accompanies your loved one at appointments, helps them stay organized, takes general notes with permission, and sends a clear summary to the authorized family contact.
An in-person or telephone visit focused on general well-being, immediate practical concerns, safety observations, food access, and upcoming appointments. Not a medical assessment or emergency service.
We help identify, arrange, or coordinate transportation for appointments, errands, and approved activities through vetted family, taxi, rideshare, or partner options.
Assistance with planning, ordering, pickup, delivery coordination, or accompaniment for groceries, pharmacy pickups, and essential errands. No medication handling.
With appropriate consent, the authorized family contact receives a written summary after each visit: date, observations, tasks completed, concerns, and follow-up items.
Non-medical support after hospital or rehabilitation discharge — checking that follow-ups are scheduled, transportation is arranged, and the family understands the written plan.

Families living abroad often need someone locally to check in, accompany a relative to an appointment, help coordinate errands, or send a reliable update home. Jamaica services will begin in selected service areas and expand as vetted coordinators become available.
Service availability, travel fees, transportation costs, and response times depend on the client's parish and location.

Florida services may include appointment companionship, wellness check-ins, practical coordination, discharge follow-up support, and family communication for loved ones in our Central Florida service area.
We only serve confirmed Central Florida service areas at this time.
